Adult Psychiatric Rehabilitation Program

Overview

The program provides rehabilitation services to adults between the ages of 18-62. Our clients are provided with structured individual counseling and group therapy sessions, designed to address one’s mental disorder/symptoms and enhance independent living skills. The concentration is to assist clients to successfully integrate back into their community to foster independent living .

Criteria for enrollment in PRP-A (adults)

 

  • Must be 18-62 years of age
  • Mut be Medicaid eligible
  • Must be in care of a therapist, psychiatrist, or any other licensed medical provider
  • Must not be currently hospitalized
  • Must not be currently incarcerated
  • Must have willingness to receive services
  • Must reside in Maryland
Scope (PRP-Adult):

 

As a participant in our psychiatric Rehabilitation Program Adults, you will receive:

  • A comprehensive mental health assessment
  • An individualized rehabilitation plan customized for your specific needs
  • 6-8 Counseling sessions per month conducted by a highly trained PRP counselor
  • Assistance with applying for and maintaining entitlement benefits
  • Assistance with identifying and/or maintaining gainful employment

  • Planning and cooking meals
  • Managing medications
  • Wellness health care promotion
  • Personal Safety
Social Skills:

 

  • Interpersonal skills

Independent Living and Community Living Skills Training:

 

  • Maintaining suitable housing
  • Utilizing community resources
  • Transportation and mobility training
  • Personal finance and budgeting
Illness Management Skills Training:

 

  • Understanding and dealing with mental illness as a disease
  • Dealing with symptoms and understanding each person unique challenges of daily living
  • Medication management/Education
  • How to recover your sense of self and your life in spite of having an illness

Case Management:

 

All participants in any of PHP programs are eligible for case management services. Case management services are designed to address a variety of challenges any program participant may have. Our goal is to provide ancillary support to program participants that encourage their successful participation in their respective program while encouraging sobriety and community independence. Case management services are ongoing throughout the individual’s enrollment in our programs and in many cases beyond.

These services include:

  • Assistance with identifying stable and/or suitable housing.
  • Assistance with physical health care management
  • Exploring eligibility for entitlement benefits to include:
    • Food stamps,
    • Comprehensive health insurance,
    • Social Security income and disability benefits
    • Assistance with identifying gainful employment and applying for job opportunities
  • Assistance with managing daily life tasks to include:
    • Appointment setting
    • Personal grooming
    • Time management
    • Personal budgeting and finance
    • Legal support and advocacy

Minor Psychiatric Rehabilitation Program

Overview

The PHP minor program, ages 7-17 is designed to assist families, caregivers, legal guardians with assistance with their loved one who Is currently presenting a dysfunction in his/her customary settings e.g. Home, school, recreational activities, work, just to name a few. In addition, the minor is displaying inappropriate behaviors, that indicates the youth is at possible risk to self, peer relationships and family members are at risk.

Our approach is to provide the youth and their families with structed individual and group counseling sessions aimed to help manage their current dysfunction or challenges the youth is experiencing. We particular focus on, anger management, Individualized education program (IEP) support, coping skills & age-appropriate life skills development. We work diligently with the youth guardian and other service providers involved in the youth’s care. The goal is to provide behavioral interventions to help the youth thrive and avoid the need for higher levels of care, expulsion from school and or loss off residential placement.

  • Must be 7-17 years of age
  • Must be in the care of a therapist, psychiatrist, or any other licensed medical provider
  • Must be Medicaid eligible
  • Must be a resident of Maryland
  • Must not be currently hospitalized
  • Must not be a resident of a psychiatric facility
Scope (PRP-Minors):

 

As a participant in our Psychiatric Rehabilitation Program for Minor, you will receive:

  • A comprehensive mental health assessment
  • An Individualized rehabilitation plan customized for your specific needs
  • 3-5 counseling sessions per month conducted by a highly trained PRP counselor
  • Approved therapeutic recreational activities
  • Skills Training Areas

Outpatient Mental Health Clinic

Medication Management

Outpatient Mental Health Clinic (OMHC)– Provides services for clients experiencing different mental health conditions help manage their symptoms and improve their quality of life. We provide services to individuals of all ages, from children to adults. Our clinic offers individual therapy, which is a core service conducted by licensed mental health professionals such as therapists, social workers, and licensed professional counselors, are all trained to help individuals cope with their symptoms and improve their mental health. Group therapy is another essential service we offer in our clinic. Our group therapy provides a safe and supportive environment where individuals can connect with others facing similar mental health challenges. It offers a platform for individuals to share their experiences, learn from others, and receive support and guidance from a trained therapist. Crisis intervention is another service offered, which involves immediate support and intervention for individuals experiencing a mental health crisis.

Medication management is extensive service provided. Our medication management team includes, Psychiatric Mental Health Nurse practitioner, and other medical professionals who work together to prescribe and manage medications for all participants. Our medical professional team are all trained to monitor medication effectiveness, manage side effects, and adjust dosages as needed to ensure optimal treatment outcomes to improve client’s quality of life, and reduce symptoms.
